Telehealth Informed Consent

This Telehealth Informed Consent ("Consent") applies to all telehealth services provided through BridgeMeds, a precision medicine platform operated by TrueEval Medical Group PLLC ("Practice," "we," "us," or "our"), a licensed medical entity. By using our telehealth services, you ("Patient," "you," or "your") acknowledge that you have read, understand, and agree to the terms outlined below.

1. Nature of Telehealth Services

Telehealth involves the delivery of healthcare services using electronic communications, information technology, or other means between a healthcare provider and a patient who are not in the same physical location. Our telehealth services may include, but are not limited to:

  • Asynchronous clinical assessments and medical evaluations
  • Review and interpretation of laboratory results and biomarker data
  • Prescription of medications, including compounded pharmaceuticals
  • Ongoing treatment monitoring and protocol optimization
  • Secure messaging with your assigned provider

All clinical services are delivered by or under the supervision of licensed healthcare providers, including board-certified providers (MD/DO) and nurse practitioners (NP/APRN), credentialed through TrueEval Medical Group PLLC.

2. Scope of Services

BridgeMeds provides telehealth-based evaluation, diagnosis, and treatment in the following areas: hormone optimization (testosterone replacement therapy, hormone replacement therapy), weight management (GLP-1 receptor agonists, metabolic protocols), peptide therapy, sexual health, mental wellness, dermatology, sleep optimization, longevity protocols, thyroid management, and at-home laboratory testing. Our services are available to patients in all 50 U.S. states and the District of Columbia, subject to state-specific regulations and provider licensure.

3. Patient Eligibility and Responsibilities

You represent and agree that:

  • You are at least 18 years of age.
  • You are physically located in the United States at the time of your consultation.
  • The information you provide in your health assessment, medical history, and demographic forms is truthful, accurate, and complete.
  • You will promptly inform your provider of any changes to your health status, medications, or medical conditions.
  • You understand that withholding or providing inaccurate information may result in inappropriate treatment recommendations and may endanger your health.
  • You will follow your provider's instructions regarding laboratory testing, medication use, and follow-up care.
  • You will not share prescribed medications with others.

4. Benefits and Risks of Telehealth

Potential Benefits

  • Convenient access to healthcare without travel or waiting rooms
  • Reduced exposure to communicable diseases
  • Access to specialized providers regardless of geographic location
  • Comprehensive biomarker analysis and data-driven treatment protocols
  • Ongoing monitoring and protocol optimization based on measurable outcomes

Potential Risks and Limitations

  • Telehealth services may not be appropriate for all medical conditions. In some cases, an in-person examination may be necessary.
  • Electronic communication systems may experience technical failures, which could delay care.
  • The absence of a physical examination may limit the provider's ability to diagnose certain conditions.
  • There is a risk that information transmitted electronically could be intercepted by unauthorized parties, despite our use of encryption and security measures.
  • Delays in evaluation or treatment may occur due to technology limitations.

5. Prescribing and Pharmacy Services

If your provider determines that medication is appropriate for your condition, prescriptions will be sent to an FDA-registered, cGMP-compliant compounding pharmacy. All medications are compounded specifically for your prescribed protocol. You understand that:

  • Your provider may prescribe medications, including controlled substances where clinically appropriate and legally permitted, based on your clinical evaluation and laboratory results.
  • Certain treatments (including testosterone therapy and other controlled substances) require current laboratory results before prescribing.
  • Compounded medications are not FDA-approved products, though they are prepared in FDA-registered facilities using pharmaceutical-grade ingredients.
  • You are responsible for reporting any adverse reactions or side effects to your provider promptly.

6. Laboratory Testing

Comprehensive laboratory testing is a core component of our clinical model. We may recommend or require at-home laboratory testing to establish baseline biomarker levels, monitor treatment efficacy, and ensure patient safety. You understand that:

  • Recent laboratory results (within 90 days) may be required before certain treatments can be prescribed.
  • At-home lab kits are provided through certified laboratory partners and analyze up to 120+ biomarkers.
  • Laboratory results are reviewed by your assigned provider and used to inform treatment decisions.
  • You may be asked to complete follow-up laboratory testing at regular intervals to monitor your progress.

7. Emergency Situations

Telehealth services are not intended for medical emergencies. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room immediately. BridgeMeds does not provide emergency medical services. Our providers are available 7:00 AM to 11:00 PM across all five U.S. time zones, 365 days per year, for non-emergency clinical inquiries.

8. Privacy and Confidentiality

We are committed to protecting your health information in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and applicable state privacy laws. Your protected health information (PHI) will be used and disclosed only as described in our Privacy Policy and Notice of Privacy Practices. All electronic communications are encrypted using industry-standard 256-bit TLS encryption.

9. Consent to Treatment

By completing a health assessment on BridgeMeds, you consent to receive telehealth services from TrueEval Medical Group PLLC and its affiliated providers. You understand that:

  • This consent is voluntary. You may withdraw consent at any time by contacting us, though withdrawal may limit our ability to provide services.
  • Your provider will review your assessment, medical history, and laboratory results before making treatment recommendations.
  • You have the right to refuse any recommended treatment.
  • You have the right to request a copy of your medical records.
  • You have the right to ask questions about your treatment at any time through secure messaging.

10. State-Specific Disclosures

Telehealth regulations vary by state. The following disclosures apply based on your state of residence:

  • All States: You have the right to be informed of the identity and credentials of any provider involved in your care.
  • California: You have the right to receive a copy of your medical records. Telehealth is subject to all California laws regarding the confidentiality of healthcare information.
  • Texas: A provider-patient relationship is established through telehealth. You may file a complaint with the Texas Medical Board.
  • New York: You have the right to refuse telehealth services at any time without affecting your right to future care.
  • Florida: Your provider is licensed to practice in the State of Florida and is subject to regulation by the Florida Board of Medicine.
  • All Other States: Your provider is licensed in your state of residence or holds appropriate interstate licensure. State-specific rights and complaint procedures are available upon request.

11. Limitation of Liability

You acknowledge that no guarantees have been made regarding the outcome of your treatment. Medicine is not an exact science, and your provider cannot guarantee specific results. You agree that TrueEval Medical Group PLLC, its providers, and BridgeMeds shall not be liable for any damages arising from the use of telehealth services, except as required by applicable law.
